Implementation of Integrated Management Systems

The focus is the integration of all business management systems into one single system.This is done as per client specific need to comply with requirements such as good governance, international standards, codes of practice, best practice, legal, corporate requirements and own company specific requirements.

We conduct a GAP Analysis against the International Standard requirements such as ISO 9001:2015, ISO 14001:2015, ISO 45001:2018, SANS Codes such as SANS 3001: Rail Safety Standard and the client’s specific needs and risks. The IMS integrates all business processes into one single management system by defining the company’s value chain, a single-entry level into easily readable flowcharts. The IMS also link all software applications used by the different departments with the client specific processes. 

Each flowchart can be automated without coding into workflows in our software application called Kelsa. No coding enables the client to revise the workflows, develop new workflows and link workflows with each other. All forms and checklists are integrated in the platform.

Once we have developed the IMS, we assist the client by implementing the system and ensure compliance to the system in order to be certified by an independent Certification Body. We also facility the Certification Audit done by an Independent Certification Body up to the point where we close-out the findings with the client.

 

We hold our client’s hand from having no systems in place, close the GAPS that exists to certification.

Contractor Management

Contractors can be assisted to develop and implement client requirements to achieve Certification and or Legal Compliance. Our pricing structure has been structured in such a way that it is affordable for small companies to achieve these requirements.

 

It also enables large companies to select, appoint and onboard contractors who are already compliant to their requirements. Contractors can hit the ground from the word go. This enables large organizations to save time and money to get contractors compliant once they have onboarded.

We follow international and well-known methodologies depending on each situation for example, to name a few:

  • Failure Mode, Effects, and Criticality Analysis (FEMECA/FEMEA)

  • Swiss Cheese Model

 

We ensure that all conclusions can be substantiated by evidence. The process that we follow has been tested and are well known in industry.

The steps used during the Root Cause Analysis are:

  • Initiate the Root Cause Analysis

  • Establish the team

  • Complete the detail of the incident

  • Establish the timeline of events

  • Review the detail of evidence collected

  • Determine the Immediate causes

  • Determine the Root Causes

  • Determine the System Failures

  • Compile an Action Plan

 

The information gathering process involved:

 

  • People Information: Statements taken of eyewitnesses and expert/knowledgeable persons will be assessed.

  • Parts Information: Examination of equipment, materials, tools, etc. involved in the incident (Photos taken, and drawings compiled will be assessed)

  • Paper Information: Scrutinizing of training records, codes and practices, maintenance records, risk assessments, etc. relevant to the incident

  • Position Information: Analysis of position of persons, equipment, materials, etc. - prior to contact / incident; during contact / incident; after contact / incident (Assess Photo taken, and drawings done)

  • Process Information: Information gathering on speed, temperature, pressure, humidity, environment, etc. at the time of the incident

 

The following were major incidents investigated the past two years:

 

  • Facilitating an Independent Root Cause Analysis on the collapse of the Reclaim Stockpile Tunnel Floor Ghana where seven people were killed. 

  • Facilitating a Root Cause Analysis on an Induction Furnace: Financial Loss; Production Downtime and Damages.

  • Facilitating a Root Cause Analysis on a Ball Mill: Vibration causing Financial Loss; Production Downtime and Damages.

  • Review the Root Cause Analysis Investigation in Botswana: Electrocution of a contractor employee by a 11kv electrical cable.

  • Review the Root Cause Analysis in Botswana: Fatal Accident of a contractor employee falling 17 m from a ladder.

  • Conduct a Root Cause Analysis in the investigation of a fatal motor vehicle accident.

Compliance review of Procedures

We facilitate the identifying and compiling/review of Standard Operating Procedures (SOP), Mandatory Codes of Practise; Codes of Practise, Safe Work Procedures, Job Safety Analysis (JSA) worksheets, Developing Guideline Documents, Inspection Documentation and Registers.

 

We ensure that the documentation is risk based, focus on the company processes, practical and user friendly. The documentation can all be integrated in our software application called Kelsa and directly linked with the corrective action module.
